A Questionable Intelligence Activity is any intelligence or intelligence-related activity that may violate the law, any Executive Order or Presidential directive, or any applicable DoD policy, including DoD Manual 5240.01.

Why this answer

The test is reason to believe, not proof: an activity is reportable as a QIA once there is reason to think it may break a law, an Executive Order, a Presidential or Intelligence Community directive, or DoD policy.
